Clinical Uses and Indications of T-Locking Plates
The T-Locking Plate is a specialized internal fixation system designed specifically for veterinary orthopedics, demonstrating unique value in repairing a variety of complex fractures:
Particularly suitable for complex comminuted scapula fractures, the 90°-shaped T-plate stabilizes both the scapular spine and the acromion. Clinical studies have shown that using a locking compression plate to stabilize the infraspinatus and supraspinatus regions of the scapula can achieve excellent fracture reduction and stability.
TPLO (Tibial Plateau Horizontal Osteotomy) Procedure: For the treatment of anterior cruciate ligament rupture in dogs, T-shaped plates provide stable osteotomy fixation. The ALPS® TPLO System, for example, is specifically designed for this procedure and comes in eight sizes to accommodate breeds ranging from toy to giant breeds.
Complex Periarticular Fractures: The T-shaped design is particularly suitable for treating fractures near joints, such as proximal humeral, distal femoral, and proximal tibia fractures, where multiplanar fixation is required.
Mandibular Fractures: Can be used to treat specific fracture types involving the teeth, and are more effective when used with a cerclage wire.
**Patellar Luxation Correction Surgery**: The T-plate from the PRCL Locking Plating System has been successfully used to treat bilateral patellar luxation with anterior cruciate ligament rupture in dogs.
This type of implant is particularly suitable for osteoporotic patients, young animals, or small dogs and cats where traditional plating is inadequate, as it does not rely on friction at the bone-plate interface for stability.
2.Intraoperative Procedure:
1).Fracture Reduction:
- Use open or closed reduction techniques to achieve anatomical or functional reduction.
- Temporarily maintain reduction with Kirschner wires or forceps.
2).Plate Contouring:
- Use a dedicated plate bending tool for three-dimensional shaping.
- For scapular spine fixation, precisely bend the T-plate 90° to accommodate the anatomy.
- Avoid repeated bending that can cause metal fatigue.
3). Plate Positioning:
- Place the crossbar of the T-plate along the scapular spine or joint margin.
- Ensure the longitudinal arm is aligned with the long axis of the bone.
- Use a plate holder to temporarily fix the position.
4). Screw Fixation:
- Initially insert one non-locking screw on each side closest to the fracture.
- Use a drill guide to ensure precise drilling (e.g., the ALPS system).
- Alternate between locking and non-locking screws.
- For TPLO procedures, use a custom drill sleeve for selective compression.
5). Final Review:
- Confirm proper locking of all screws.
- Check joint range of motion and fixation stability.
- Obtain intraoperative X-rays to confirm reduction and implant position.
